At Godswell Physiotherapy we are committed to protecting and respecting your privacy. This Privacy Policy tells you how we use this personal information collected in our clinic.

Your personal data is data which by itself or with other data available to us can be used to identify you.  We are Godswell Physiotherapy Ltd, the data controllers. You can contact our Data Protection Officer at Godswell Lodge, Church Street, Bloxham, Oxon OX15 4NB.

What type of information is collected from you?

When you book an appointment at Godswell Physiotherapy we ask you for your full name, date of birth, postal address, telephone number, email address, your GP surgery and certain other personal information.

We will retain details and clinical notes relating to the care you receive within our practice and from third parties such as GPs, consultants, insurance companies.  These are legal medical notes and, therefore, after the last date of treatment, we are obliged by law to keep them for:

-       8 years for an adult

-       Until the age of 25 for a child under 16 years

How is your information used?

We will use information about you to enable us to arrange appointments with the appropriate clinician and share this knowledge with other Godswell Physiotherapy colleagues when appropriate and with your consent.

We will keep and store your medical records as stated above so we have knowledge of your condition and previous episodes.

We will not disclose your personal data, including records of your treatment, unless required to do so by law or with your explicit consent for purposes such as:

-       referral to other healthcare professionals such as consultants or GPs

-       dealing with requests from legal firms if relevant

-       clinic audit to ensure our processes and care are kept to a high standard

-       to provide and fulfil services between yourself and Godswell Physiotherapy, and that of your insurance provider if using one

-       accountancy purposes

-       Physiotec – our online personal exercise programme

Who has access to your information?

We will not sell or rent your information to third parties.

We will not share your information with third parties for marketing purposes.

How is your information stored?

All written medical notes are stored at Godswell Physiotherapy in locked fireproof cabinets; they are destroyed after the legal amount of time, as stated previously.

Personal information stored on our online diary is again stored for the legal amount of time and then deleted.

Any personal information sent to insurance companies via email is carried out by using encryption via Egress Switch.

Physiotec, our online exercise programme, stores (with your consent) your name and email address so we can send you a personalised exercise programme if required/requested. This uses cloud-based storage which is UK based and Physiotec are also compliant with GDPR.

Emails and any letters written on computer are printed and kept with the written medical notes, the computer is cleared of this data on a monthly basis. All computers are password protected.
